Splenic cysts are subdivided into several groups: (1) congenital, including the epidermoid cyst; (2) parasitic, including the echinoccal cyst; (3) neoplastic, including the dermoid and epithelial cysts; (4) pseudocystic, including cysts associated with adult polycystic disease and occult splenic rupture.
1 Clinical symptoms and laboratory test results
Most cysts are asymptomatic. Large or multiple cysts may produce splenomegaly and/or left upper quadrant pain or pressure. Cysts related to bacterial infection typically are associated with an elevated WBC.
2 Sonographic findings
True cysts, such as epithelial cysts, typically display smooth, thin walls, an anechoic interior, and distal acoustic enhancement. Echinoccal cysts and pseudocysts usually demonstrate a thicker wall that may become calcified, and may contain internal echoes caused by hemorrhage, infection, or necrosis.
3 Conditions that can mimic splenic cysts
Old hematomas resulting from occult trauma may appear to meet the full criteria of a simple neoplastic cyst.
4 Where else to look
The presence of multiple splenic cysts suggests adult polycystic disease and the sonographer should examine both kidneys for evidence of cyst formation. Echinoccal infection of the spleen is rare. Nevertheless, the sonographer should examine the liver if this pathology is suspected.
